The Department may disclose my Personal
Information to other Australian Government agencies, including those located in
States and Territories outside New South Wales.
The above government agencies may use my
Personal Information for any purpose relating to the exercise of their
government functions, including but not limited to the evaluation and
assessment of my training, the determination of my eligibility to receive
subsidized training or for any Fee Exemptions or Concessions. My Personal Information may also be disclosed
to other third parties if required by law.
I consent to the collection, use and
disclosure of my Personal Information in the manner outlined above.
I also acknowledge and agree that the
Department may contact me by telephone email or post during or after I have
ceased subsidized training with Australian Concert and Entertainment Security
Pty Ltd trading as Affirm Training for the purposes of evaluating and assessing
my training.
I declare that I have been provided
with or access to online information pertaining to the course I am enrolling in
which includes:
a) the code, title and currency of the
training product
b) the training and assessment, and related
educational and support services in the supplied marketing brochures (or ACES
Workplace Rostering System):
i) estimated duration
ii) expected locations at which it will be provided
iii) expected modes of delivery
iv) name and contact details of any third party that will provide training
and/or assessment (if applicable)
v) any work placement arrangements
vi) any additional fees or equipment I maybe required to provide or purchase as
part of the training program
c) the RTO’s obligations to the learner,
including that the RTO is responsible for the quality of the training and
assessment in compliance with these Standards, and for the issuance of the AQF
[Australian Qualifications Framework] certification documentation in the
supplied marketing brochures (or ACES Workplace Rostering System):
d) the learner’s rights, in the supplied
marketing brochures (or ACES Workplace Rostering System) including:
i) details of the RTO’s complaints and appeals process
ii) if the RTO, or a third party delivering training and assessment on its
behalf, closes or ceases to deliver any part of the training product that the
learner is enrolled in.
e) the learner’s obligations in the
supplied marketing brochures (or ACES Workplace Rostering System) including:
i) in relation to the repayment of any debt to be incurred under the VET
[Vocational Education and Training] FEE-HELP scheme arising from the provision
of services
ii) any requirements the RTO requires the learner to meet to enter and
successfully complete their chosen training product
iii) any materials and equipment that the learner must provide
f) information on the implications for the
learner of government training entitlements and subsidy arrangements in
relation to the delivery of the services in the supplied marketing brochures
(or ACES Workplace Rostering System) including:
g) all relevant fee information in the
supplied marketing brochures (or ACES Workplace Rostering System) including:
i) fees that must be paid to the RTO
ii) payment terms and conditions including deposits and refunds as detailed in
the marketing brochures (or ACES Workplace Rostering System) or access to the
RTOs online policies
h) the learner’s rights as a consumer,
including but not limited to any statutory cooling-off period, if one applies
i) the learner’s right to obtain a refund
for services not provided by the RTO in the supplied marketing brochures (or
ACES Workplace Rostering System) should:
i) arrangement is terminated early
ii) the RTO fails to provide the agreed services.
j) that not providing a USI number will
result in no nationally recognised certifications being issued
k) that I am not an Overseas Student and I
have the correct VISA to attend a nationally recognised training program.
(International students are obligated to ensure their VISAs allow them to study
in Australia).
l) have provided the RTO with evidence of
my current VISA and conditions if not an Australian or New Zealand Citizen.

Rostering System) including:
i) Payments and Refunds
ii) Complaints and Complaint Documentation
iii) Grievances and Appeals
iv) Training Policy (ACES Group Employees Only)
v) Training Policy (External Clients)
n) and the RTOs terms and conditions as
detailed in the Enrolment Form
o) I furthermore consent that my
information may be released to third parties where the RTO is obligated to
report on my enrolment and completion of training and assessment practices
which may include photographs to develop photo identification cards.
p) I understand that I may be recorded with
visual and audio devices for safety, compliance, and marketing purposes
throughout the duration of my training and assessment and will advise an RTO
representative if I do not want to partake in the course due to this.
